Cleanroom Lab Access Protocol

Please read the following lab access procedures carefully!

***All cleanroom new user training is canceled until further notice due to proactive policies instituted by the University amid the global COVID-19 pandemic.***

Failure to complete each step of these instructions could prevent or delay your access to the Cleanroom Laboratories within MNTL.

Step 1: Submit Key Request Forms

  • Keycard Request Form

    Complete and submit the Keycard Request Form online. You can also obtain a print copy from the MNTL Business Office in Room 2104 MNTL. Your advisor must email the business office stating that you can take the training.

    Online key card request form and MNTL biolab questionnaire must be submitted by Friday 2:00pm before the training day.

  • Cylinder Key Request Form (if needed)

    Print and submit the Cylinder Key Request Form to the Business Office (if needed.) Make sure it is signed by your PI.

Step 2: Complete Online Forms and Training Session Certificates

  • Complete the MNTL Questionnaire for New Cleanroom Users

    Access the New Cleanroom User Questionnaire

    MNTL Cleanroom questionnaire must be submitted by Friday 2:00pm before the training day.

  • Take the online General Lab Safety Training

    Take the online General Lab Safety Training offered by the University’s Division of Research Safety (DRS).

    If you are an external user, please send e-mail to for DRS login and password information. Please submit the certificate that you receive after this training.

  • DRS Online HF Safety Training

    Take the DRS online HF Safety Training program. Please submit the certificate that you receive after this training.

Step 3: Attend MNTL Safety Trainings

  • MNTL Cleanroom Lab Orientation and Safety Training

    MNTL Cleanroom Lab Orientation and Safety Training is normally held from 2:00 p.m. until 3:30 p.m. on designated dates each month. Report to Room 2104 at the MNTL building no later than 1:50 p.m. when you first arrive for the training; you will sign in here. Following this, you will next proceed to the training area, typically Room 3003, unless otherwise noted. Your submitted Cleanroom forms are your confirmation that you want to attend the training.

    All the certificates must be submitted by 2PM the Friday before the training is set to occur — preferably by email to or drop off at Room 2104.

  • MNTL Building Safety Training

    MNTL Building Safety Training is held 3:30-4pm, right after the cleanroom safety training, on the same days listed above, report to Room 2104 MNTL at 3:30 p.m. for the training. Your submitted Keycard Request Form and General Lab Safety Training Certificate are your confirmation that you want to attend the training.

Step 4: Attend MNTL Cleanroom Walkthrough

Step 5: Obtain a Proximity Card and (Optional) Cylinder Keys

  • Proximity Card Form

    You can obtain a Proximity Card Form no sooner than one day after your training. If you already have a 5-digit prox card number on the back of your i-card, you do not need to get another prox card number. If you do need a prox card number, the form is $8.50, payable by cash or check (no debit or credit cards).

  • Cylinder Keys

    You will be able to get cylinder keys after you have obtained your prox card number. Each key requires a $25 refundable deposit, payable by cash or check.

Step 6: Tool Training and Access

  • Schedule training with an engineer on a particular tool to become an approved user. This is required before using a tool by yourself. After you are a user, you will be able to reserve a tool and receive information about tool status and availability. Training is required for any use of HF or Piranha etch. Please submit the online training request form to request training. If you would like to discuss with the staff before request training, you can contact the appropriate engineer listed on the Cleanroom Equipment Super-User List.

Additional Access Information

Once the above access procedures have been finalized, cleanroom users should contact the appropriate “super-user” to schedule individual training on the processing equipment within the cleanroom labs. (See Cleanroom Equipment Super-User List.) Also be aware that anyone using the MNTL cleanrooms after hours is required to follow the laboratory's Buddy System Policy. If you plan to use hydrofluoric acid (HF) in any concentration in MNTL cleanroom, please fill out this online form. Our staff will contact you to provide additional training for HF usage in MNTL.
